If you are applying for Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI) benefits, remember the following “Social Security Disability Checklist” as you submit your application.
- Compile a list of all diagnoses and conditions you have that prevent you from working.
- Include a list of all treating healthcare providers. This includes all emergency room visits and inpatient hospital stays. You typically want to tell SSA about all healthcare providers that you are currently seeing. Also include any providers you saw during the prior year. Including addresses and phone numbers is helpful.
- Include an accurate list of all jobs you have performed in the last fifteen years. You will want: the dates you worked, what employer you worked for, what your job title was, and what your job duties were. You will also want to include the physical requirements of the job. Physical requirements include standing, sitting, walking, lifting, kneeling, crouching, and crawling.
- You need to include information about marriages (current and past) and minor children. This is because there may be additional benefits you can receive if you have minor children. A current or prior marriage may also make you eligible for different Social Security benefits.
- If you have served in the military, include your dates of service, your branch of service, and your rank at discharge. Provide your DD214 discharge paper can also be helpful.
